Job Description

Provides skilled physical therapy to patients in the home. Review patient progress with appropriate outcome, as needed.
Notify the team leader of any patient problems, concerns or issues as they occur. Review patient progress with the team leader during case discussions.
Assess and identify patient’s needs and problems to determine the patient’s need for care and services. Assess the physical capabilities of the patient through testing and functional abilities. Assess the home environment for safety.
Discuss plan and prioritizes patient needs with the patient/caregiver. Update the care plan as the health status of the patient changes.
To provide clinical inputs for co mprehensive assessment / treatment / discharge.
Maintain patient confidentiality and demonstrates respect for patient and patient property.
Documentation of the patient clinical notes, progressions and treatment.
To demonstrate high level of clinical effectiveness by use of evidence based practice.
To educate and provide necessary training to patient family members, caretakers.

Relevant Experience: 0-2 Years

Education

Graduation in Physiotherapy
